Notice Concerning the Employee Disability Questionnaire

The SPB has developed a new scannable Employee Disability Questionnaire, and has distributed a limited supply to state departments for new employees. The new form will soon be available for state departments to order in larger quantities from the Office of State Publishing. Departments should no longer use the old Form 131 or Form 131a. The SPB no longer has the capability to enter data from these forms into the employee database. All general resurveying to update information on departmental representation of employees with disabilities should be postponed until further notice by the SPB.

The Statewide Disability Advisory Council (SDAC) meets on the first Wednesday of each month, in Room 435, at the Resources Building. Attendance is open to departmental Disability Advisory Committee (DAC) members, and EEO Office staff. Issues affecting employees with disabilities in state service are discussed and actions are proposed to the SPB to improve equal employment opportunity and promote non-discrimination. (See also Disability Information Page) The California Civil Rights Officers' Council (CCROC) meets every-other-month to discuss EEO issues and to share the latest EEO information. Departmental EEO Officers are highly encouraged to attend and participate in the monthly meetings.
